Semaglutide & Tirzepatide Dosing Questions: Missed Doses, Injecting Early, and Titration

If you take compounded semaglutide or tirzepatide, dosing questions come up constantly: What if I miss a dose? Can I inject a day early? Can I stay at my starting dose longer, or move up more slowly? This guide answers the most common dosing questions in plain language. It is educational only — your Ondra provider, through Wasef Health, PC, manages and approves every dose change based on your individual response. Never change your dose without provider approval.

What if I miss a dose?

Semaglutide and tirzepatide are taken once weekly. If you miss your scheduled injection, what to do depends on how close your next dose is.

The general rule

If your next scheduled dose is more than 2 days (48 hours) away, take the missed dose as soon as you remember, then continue your regular weekly schedule. If your next dose is within 2 days, skip the missed dose entirely and resume your normal schedule. Never take two doses at once or on back-to-back days to "make up" for a missed one.

A single missed dose will not undo your progress or force you to restart titration. If you miss more than one week in a row, do not resume at your previous dose without checking in — your provider may want to step you back down to reduce the chance of side effects. Message your provider through the patient portal if you're unsure.

Can I inject a day early?

Shifting your injection day to better fit your schedule is common and generally acceptable when done in small amounts. Moving your shot 1–2 days earlier or later to accommodate travel, work, or a special occasion is usually fine, and once you've shifted, that becomes your new weekly anchor day.

What's not recommended is injecting significantly early — for example, taking your next dose after only 4 or 5 days — because it shortens the interval between doses and can stack medication in your system, raising the risk of nausea and other side effects. These medications are designed around a 7-day cycle. If you need to change your injection day by more than a couple of days, message your provider first.

Can I stay at 2.5 mg (or my starting dose) longer?

Yes — and for some patients, staying put is the right call. The standard tirzepatide progression starts at 2.5 mg weekly and titrates up toward a maximum of 15 mg; semaglutide starts at 0.25 mg and titrates toward 2.5 mg. But titration is not a race, and the "lowest effective dose" is a legitimate goal.

If you're losing weight steadily and feeling well at 2.5 mg, there may be no reason to move up. Many patients maintain results at lower doses. If you'd like to stay at your current dose rather than follow the default increase, tell your provider through the portal — they can hold you there and adjust the plan.

Can I titrate up more slowly?

Yes. Slower titration is a common and reasonable approach, especially if you experience nausea, fatigue, or other side effects when your dose increases. Rather than following the standard monthly step-up, your provider can extend the time at each dose, giving your body longer to adjust before the next increase.

This is a clinical decision made between you and your provider — it's not something to change on your own by splitting or altering doses. If side effects are making titration difficult, message your provider through the patient portal and describe what you're experiencing. They can slow your schedule, hold your dose, or adjust your plan.

What happens if I miss a weekly dose of semaglutide or tirzepatide?

If your next scheduled dose is more than 2 days away, take the missed dose as soon as you remember, then resume your normal weekly schedule. If your next dose is within 2 days, skip the missed dose. Never take two doses to make up for one. A single missed dose will not reverse your progress. If you miss more than one week, message your provider before resuming.

Can I inject my GLP-1 medication a day or two early?

Shifting your injection by 1–2 days to fit your schedule is generally acceptable, and that day becomes your new weekly anchor. Injecting significantly early — after only 4 or 5 days — is not recommended, because it shortens the interval between doses and can increase side effects. These medications are designed around a 7-day cycle.

Can I stay at 2.5 mg of tirzepatide instead of increasing?

Yes. If you're losing weight steadily and tolerating the medication well at 2.5 mg, staying at that dose can be appropriate. Titration toward higher doses is not mandatory. Tell your provider through the patient portal that you'd like to hold your current dose, and they can adjust your plan.

Can I titrate up more slowly to reduce side effects?

Yes. Your provider can extend the time you spend at each dose before increasing, which gives your body longer to adjust and can reduce nausea and other side effects. This is a clinical decision — message your provider through the portal rather than altering doses yourself.
