Semaglutide Side Effects: What’s Normal and What to Watch For

Semaglutide is highly effective, but it comes with a learning curve. Most side effects are manageable and temporary — here’s how to navigate the adjustment period.

Key takeaways

✔

Nausea is the most commonly reported side effect and usually improves within the first few weeks.

✔

GI side effects like nausea, diarrhea, and constipation are usually mild to moderate and often peak after dose increases.

✔

Rare but serious risks include pancreatitis and gallbladder problems, which require prompt medical attention.

✔

Simple changes to food choices, meal size, and hydration can make side effects much easier to manage.

Why semaglutide causes nausea

Semaglutide slows gastric emptying, which means food moves more slowly from your stomach into your small intestine. That delayed movement is part of why the medication helps reduce hunger, but it can also create the queasy feeling many people notice early on.

The effect is dose-dependent. At lower starting doses, nausea may be minimal. As the dose increases, symptoms can flare temporarily before improving again as your body adjusts.

The most common side effects

Nausea

Most common early in treatment and after dose increases. Smaller meals, bland foods, and avoiding lying down right after eating can help significantly.

Diarrhea

This can happen during the adjustment phase, especially after titration. Staying hydrated and temporarily avoiding heavy or high-fat meals can reduce flare-ups.

Constipation

Some people shift from loose stools to constipation as the gut adjusts. Water, fiber, and regular movement usually help.

Fatigue

Fatigue is often related to eating less overall, dehydration, or lower protein intake rather than the medication alone.

Side effects related to weight loss itself

Semaglutide can lead to meaningful weight loss, and rapid loss sometimes brings its own issues, including muscle loss when protein intake is too low and visible changes in facial fat distribution.

These effects are not unique to semaglutide. The best way to reduce them is to keep protein intake high, do regular strength training, and avoid overly aggressive weight loss when possible.

Rare but serious risks

Pancreatitis is rare but important to know about. Severe upper abdominal pain, especially if it radiates to the back, should be treated as urgent.

Gallbladder issues, including gallstones, can also occur. Right-sided abdominal pain after meals, especially fatty meals, deserves prompt evaluation.

Semaglutide should not be used in people with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2.

When to contact your provider

Contact your provider if side effects are severe, persistent, or making it difficult to eat, drink, or function normally. It’s common for providers to slow down dose escalation if needed.

In some cases, supportive medications like anti-nausea treatment may help you get through the adjustment period more comfortably.
