Semaglutide While Breastfeeding: Is It Safe?

Current guidelines advise against taking semaglutide while breastfeeding — but the evidence is limited. Here's an honest look at what we know and how to manage your weight safely postpartum.

What the current evidence shows

A small 2024 study followed eight breastfeeding mothers taking low-dose semaglutide injections. Semaglutide was not detected in breast milk at any point tested. However, the study was very small, doses were lower than typical weight loss doses, and babies were not followed from birth.

Animal studies on Ozempic® and Wegovy® show semaglutide does pass into rat milk, though that does not necessarily mean the same happens in humans.

The bottom line is that we simply do not have enough evidence to say it is safe. Most clinicians and the drug’s labeling advise against use while breastfeeding.

Why the uncertainty matters

Even if semaglutide passes into breast milk in very small amounts, we do not know how those trace amounts would affect a breastfed infant’s appetite, growth, or GI function.

Separately, rapid weight loss — regardless of how it is achieved — can reduce milk supply and alter the nutritional composition of breast milk. This is a concern independent of the medication itself.

Managing weight safely while breastfeeding

Prioritize protein and fiber

Lean proteins like chicken, eggs, fish, and tofu, along with fiber-rich foods like fruits, vegetables, legumes, and whole grains, can support satiety and gradual weight loss without compromising milk production.

Don’t rush weight loss

Slow, steady progress is generally healthier for both you and your milk supply than aggressive calorie restriction or rapid postpartum weight loss.

Breastfeeding itself helps

Breastfeeding can contribute to modest postpartum weight loss benefits over time, even without extreme dieting.

Gradually increase movement

Once cleared by your provider, starting with low-impact activity like walking and yoga can help. Over time, resistance training may support muscle retention and overall metabolic health.

When can you start semaglutide after breastfeeding?

There is no universal guidance on exactly when it is safe to start semaglutide after stopping breastfeeding. Your provider can make a personalized recommendation based on your health history, goals, and how long ago you stopped nursing.

If you were taking semaglutide for type 2 diabetes before pregnancy, your provider can also discuss whether to restart it or use an alternative that is better studied in breastfeeding populations.
